  • Overview

    Organizes and conducts prescribed programs with adults and/or children in the home setting to restore function and prevent disability following disease or injury.  Helps patient reach his/her maximum performance; assume a place in society while learning to live within the limits of his/her capabilities.

    Responsibilities

    • Accurately develops and plans programs involving mobility training, therapeutic exercise, muscle re-education, fall prevention and joint coordination activities to assist the patient in regaining physical function, to relieve pain, and/or to achieve their maximum possible performance.
    • Provides accurate, complete and timely documentation related to each patient according to agency guidelines and third party payer requirements.
    • Recommends, orders, modifies, and adjusts appropriate DME to meet the specific needs of the patient population.
    • Participates as agency liaison for the training of agency employees, Mid Coast employees and community partners.
    • Participates in monitoring, evaluation, and improving the quality and appropriateness of patient care, participating in the QI program and suggesting constructive improvements.

    Qualifications

    • Maine State licensure
    • Entry level degree in Physical Therapy; one year experience in supervised setting preferred
    • Strong interpersonal skills
    • Excellent composition skills
    • Ethical reasoning
    • Valid Maine driver's license and own transportation
