  • Overview

    In collaboration with the Hospice Director, Medical Director and Interdisciplinary Team, the nurse practitioner will be responsible for conducting Face to Face visits in the patient's home or facilities for hospice recertification. The nurse practitioner will also provide appropriate patient/family/caregiver teaching as needed; promote quality services in accordance with established agency policies and procedures; ensures compliance with federal, state, and local guidelines for all services; and assess patients for eligibility for the Medicare/Medicaid Hospice Benefit.


    • Assess patient for continuation of the Medicare Hospice Benefit for certification or recertification.
    • Evaluates clinical evidence for eligibility and provides this information to the Medical Director and hospice team.
    • Document assessment finding and attestation in the patient's clinical record.
    • Providing evidence of progression of the end-stage-disease process.
    • Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues.
    • Collaborates with the Hospice Director and Medical Director.


    • Graduate of an accredited school of nursing.
    • Successful completion of an approved Masters prepared Nurse Practitioner program.
    • Licensed State of Maine as a Registered Professional and Advanced Practice Nurse.
    • ANA Certification or other national certifying organization acceptable to the Maine State Board of Nursing.
    • Two years of experience as a Nurse Practitioner.
    • Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation.
    • Current CPR certification required.
    • Experience in Palliative Care and/or Hospice preferred.
    • Knowledgeable regarding the interdisciplinary team concept.
    • Knowledge of Hospice Medicare face to face regulations related to certification and recertification.
