  • Overview

    The RN Care Coordinator will be responsible for coordinating care related to the needs of the patients and residents on the Bodwell and Mere Point communities. This includes conveying important clinical information to all staff involved with patient care and infection control; while assisting the nursing department by providing on-the-floor support for both Bodwell and Mere Point units.   The RN Care Coordinator demonstrates positive customer service and clinical expertise through effective communication, resident/patient advocacy, and collaborative relationships with all members of the clinical services team.

    Responsibilities

    General nursing responsibilities

    • Serves as a communication liaison, conveying important clinical information to all team members involved in patient/resident care.   This may be done through staff huddles.
    • Serves as a patient/resident advocate, and champion of resident rights.
    • Coordinates re-certifications for providers.
    • Performs skin assessments/wound rounds for SNF and LTC.
    • Contributes information to the Director of Nursing for patient/resident care plans.
    • Oversight of RN preceptor program in conjunction with the Director of Nursing.
    • Participates in clinical education in conjunction with the Director of Nursing.
    • Reviews and modifies C.N.A. assignments to meet resident’s needs.
    • Provides on-the-spot coaching/feedback/training to C.N.A.’s as needed.
    • Assists with meals - providing support in both dining rooms, passing trays, and feeding patients/residents who need assistance.
    • Attends C.N.A. staff meetings when possible, and contributes to the agenda.
    • Works on the floor when inadequate staffing, call-outs, or high acuity needs require it.
    • Participates in the weekday on-call nurse rotation.

    Skilled nursing responsibilities

    • Assists with admissions and discharges.
    • Participates in family communications and follow up.
    • Participates in AMA (leaving against medical advice) investigation and follow up.
    • Provides oversight of shift to shift communication.
    • Participates in weekly utilization reviews with the clinical team to discuss patient progress towards discharge goals.
    • Works in collaboration with the MDS Coordinator and the Director of Clinical Operations on tracking quality measures and providing staff education related to them.
    • Works in collaboration with the MDS Coordinator on nursing assessments for the purpose of the MDS, as well as identifying change in condition.

    Infection Prevention

    • Responsible for implementing the facility’s Infection Prevention and Control Program (IPCP) which is designed to provide a safe, sanitary, and comfortable environment and to help prevent the development and transmission of communicable diseases and infections.
    • Oversight and implementation of infection prevention and control education.
    • Track and monitor all incidents of infection by category and report findings to the Director of Nursing on a monthly basis.
    • Participate as a member of the facility’s Quality Assurance and Performance Improvement committee and report on the IPCP on a regular basis.
    • Work with the Director of Clinical Operations and Compliance to establish written standards, policies, and procedures for the Antibiotic Stewardship program.
    • Collaborate with the Pharmacy Consultant to establish an antibiotic stewardship program that includes antibiotic use protocols and a system to monitor antibiotic use.
    • Maintain a system for recording incidents identified under the facility’s IPCP and the corrective actions taken by the facility.
    • Work with the Director of Clinical Operations and Compliance to conduct an annual review of the Infection Prevention and Control Program and update the program, as necessary.

    Qualifications

    • RN License, State of Maine
    • Bachelor’s degree preferred
    • Leadership experience preferred
    • Demonstrates excellent clinical nursing skills
    • Organizational and interpersonal skills required
    • Computer experience required
    • Current CPR BLS Healthcare Provider certification required
