Overview

The Hospice Clinical Manager oversees day to day patient and clinical operations, scheduling and coordination of all clinical services to ensure quality patient care, good community public relations, and the integration of agency and departmental objectives.

Responsibilities

  • Provides direction and general oversight to Hospice Clinical Team, including Nursing, Social Work, and Home Health Aides.
  • Ensures a high quality of patient care that is delivered and administered in an efficient and cost effective manner.
  • Works collaboratively with CHANS Department managers in areas of Skilled/Palliative, Intake, Finance, HIM, general documentation, regulations and agency operational processes.
  • Maintains effective interdisciplinary relationships with internal and external resources
  • Responsible in coordinating staff schedules and patient visits to meet timely admissions and visits of Hospice clients.
  • Assures that patient care and documentation meet licensing and regulatory requirements.
  • Participates in the development of the Hospice program continuing education and development plans for clinical and administrative staff.
  • Collaborates with the Director of Hospice in the development/modifications of the Hospice program’s policies and procedures.
  • Participates in community outreach programs as needed to educate and advocate CHANS Hospice Program Services.
  • Provides information to billing department for approval of reimbursement for Hospice services, DME and medications.
  • Keeps statistical data as needed.
  • Develops and makes modifications to clinical practice standards and competencies when required.
  • Develops program goals and objectives with Hospice Director and clinical team.
  • In coordination with the Hospice Director, participates in opportunities to educate community and referral sources, etc.
  • May be required to provide direct patient care to ensure adequate coverage of patients in accordance to the Hospice Staff RN job description.
  • Keeps abreast of EMR system, issues, and updates in order to trouble shoot clinical issues around EMR.
  • Coordinates IDT meetings and responsible for tracking of actual and potential clients.
  • Manages Enclara Pharmacia and assists staff with training and support of Enclara
  • May need to cover RN on-call responsibilities when staff unavailability occurs.
  • Responsible for maintaining clinical skills/competencies (wound care, IV skills, VAC dressings) in order to support skilled nurses and skilled department
  • Participates in licensing, certification and accreditation surveys as needed.
  • Monitors licensing and certification of clinicians to ensure compliance.
  • Keeps current and knowledgeable about Hospice regulatory issues.
  • Participates in Continuous Quality Improvement activities in collaboration with the Hospice Director and QAPI Manager.
  • Participates in selection and onboarding of new staff. This includes interviewing, hiring and ensuring that all new staff receive thorough orientation, training and precepting.
  • Provides leadership, coaching and development to direct reports. Participates in and coordinate with Hospice Director on completing staff performance documentation.
  • Provides for staff development to include Hospice in-services.

Qualifications

  • RN license, State of Maine; BSN required or currently enrolled in BSN Program to complete within 2 years.
  • Hospice Certification within first year in the role.
  • Excellent communication and interpersonal skills with patients, nurses, physicians, families, and general public.
  • Demonstrated belief in philosophy of Hospice.
  • Demonstrates knowledge of Hospice Regulations.
  • Demonstrated organizational, leadership and interpersonal skills.
  • Minimum of two years leadership/management experience in healthcare preferred.
  • Current CPR BLS Healthcare Provider certification.
  • Clinical experience and expertise necessary, a minimum of five years in home care, medical-surgical or critical care setting. Home Health experience preferred.
