RN Health and Wellness Coordinator: Be part of the leadership team at Thornton Oaks, an Independent Living retirement community.  You must be a registered nurse who enjoys working with seniors. The Coordinator serves as a health resource and heads up our wellness activities for our resident retirees .

 

This is a full-time, benefited position—no weekends and no evenings. You’re part of a 7-person management team that leads a successful and dynamic retirement community.

 

The Health and Wellness Coordinator (HWC) is responsible for promoting holistic wellness of residents of Thornton Oaks by coordinating and integrating all aspects of a healthy lifestyle and by initiating a proactive approach to health care through education, training, and awareness. The Health and Wellness Coordinator supports residents through transitions of care, working closely with residents and their families as changes in level of care occur.   The Health and Wellness Coordinator is responsible for the management of the overall health and wellness planning to meet residents’ needs, assisting with the coordination of services required for resident care, planning health related educational seminars, conducting health related programs, and providing information on local health resources to residents and staff.

 

Responsibilities

  • As a key member of the marketing team, develops positive and therapeutic relationships with residents and their family from the beginning of their TO experience
  • Assists residents and families through transitions of levels of care, demonstrating knowledge and skills in communicating options for safety and quality of life through developing trusting relationships
  • Works closely with the Mid Coast Senior Health Center and Mid Coast Hospital team during transitions of care, helping to guide and communicate options with residents and families in a manner that supports and promotes their transition when they are no longer independent
  • Reviews Health History forms submitted by applicants and performs physical and functional assessments as necessary
  • Monitors resident’s health status through regular contact and/or reports from other staff. Maintains current medical information on each resident as permitted by the resident. Confidentiality is maintained
  • Maintains regular contact with residents at risk or recuperating from illness/surgery
  • Assists with coordinating delivery of health services required for individual residents utilizing appropriate agency or provider
  • Provides personal consultation with residents as needed to offer information and referral services
  • Provides assistance to residents in completing File of Life forms and contact information forms upon move-in. May assist with completing insurance claims or health related forms as needed
  • Provides Brunswick Police Department with current digital photo and demographic information on those residents with memory impairments who drive or fitness walk
  • Upon request from resident and/or family, arranges for functional geriatric assessment (at the resident’s expense) to evaluate the individual’s ability to live independently
  • Informs Executive Director and Thornton Oaks staff in a timely manner of a death in the community and ensures that permission to release that information to the community has been obtained from next of kin/POA
  • Conducts periodic health screening tests for members of the Thornton Oaks community
  • Plans and conducts educational programs promoting health and wellness.
  • Provides members of the Thornton Oaks community with written materials and/or appropriate resources on pertinent health issues
  • Promotes participation in the Running Start program
  • Maintains healthy lifestyle awareness utilizing the Stay-Well Stairwell
  • Provision of emergency response while on campus
  • Serves as resource to care providers in cases of medical emergency
  • Secondary resource for Lifeline, back up for Resident Services Coordinator in this area

Qualifications

  • Must be an RN with current Maine license to practice
  • BSN perferred
  • 2 years' experience in medical-surgical nursing practice
  • Experience in working with an oler population, some training in geriatric care perferred
  • Experience and/or ability to teach or to organize educational seminars
