Overview

Responsible for providing physical therapy treatment to clients at home and in assisted living settings under the supervision of the physical therapist, promoting rehabilitation according to agency policies.

Responsibilities

  • Provides appropriate treatment, following the plan of care as set up by the Physical Therapist.
  • Meets treatment objectives using therapeutic exercise, muscle re-education, mobility training, adaptive equipment and other modalities/treatments as set up in the POC
  • Effectively teaches/instructs the client and/or family regarding the client’s responsibility in implementing the POC. Leaves a clearly written program with client to reinforce instruction.
  • Consistently collaborates with client and/or family in determining treatment effectiveness, comprehension and reaction to treatment. Notifies the Physical Therapist of any problems or physiological changes.
  • Provides accurate, complete and timely documentation related to each client according to agency guidelines and third party payor requirements.
  • Assesses client’s skill level and suggests/adapts DME accordingly.
  • Re-assesses functional outcomes of client’s performance with the Physical Therapist.

  • Maintains ongoing communication within and beyond the facility.
  • Establishes diaglog with outside resources to maximize patient’s success.
  • Participates as agency liaison in the training of agency employees and in the education of the community to the role and function of physical therapy
  • Participates in monitoring and improving the quality and appropriateness of patient care, participating in the QI program and suggesting constructive improvements in the services offered.

Qualifications

  • Maine State licensure
  • entry level degree from accredited Physical Therapy Assistant school;
  • one year experience in supervised setting
  • strong interpersonal skills
  • excellent composition skills
  • ethical reasoning
  • Prior Keyboard/typing experience for efficient use of laptop when inputting patient data at Point of Care
  • valid Maine driver’s license and own transportation
