Overview

The Community Health and Wellness (CH&W) Coordinator  is responsible for the coordination of Community Health and Wellness programming, community outreach, and wellness education. This coordinator will also serve as the CH&W liaison for Mid Coast Medical Group.  Coordination includes relationship building, recruitment, support and mini grant oversight.

Responsibilities

  • Directs the day to day operations of all wellness programming including but not limited to Let’s Go 5-2-1-0, Weight loss and Healthy Eating, DPP, Asthma education, Tobacco Cessation, MBSR, Food for Health, and Lifestyle Choices.  Duties include scheduling, referral management, data management, and patient communications.
  • Coordinates programs operating within CH&W, such as Hannaford work-site program, BIW Biometric program and FHC Biometric/Coaching program.
  • Facilitates and maintains communication with program Medical Directors, Director of Community Health Improvement, medical staff, nursing staff and other hospital and community leaders to identify community needs for health education, disease prevention and wellness programs.
  • Teaches Diabetes Prevention Progra.
  • Performs Tobacco Cessation coaching.
  • Acts as a point person and is responsible for providing patient education and program materials to the MCMGs.
  • Work collaboratively with Community Health staff and partners to support school and childcare health initiatives and needs in an efficient, coordinated manner.
  • Represents the Community Health Program at community events and meetings, as assigned. 

Qualifications

  • Bachelor’s degree in community health, public health or other allied health field.  Masters in Public Health or related experience, preferred. 
  • Skills in program development, implementation, delivery and coordination.
  • Proficient computer skills.
  • Strong interpersonal communication and the ability to work both independently and as part of a team with enthusiasm, initiative and creativity.
  • Must be organized and detail-oriented, able to manage multiple tasks, and prioritize.
