Overview

The Wellness Nurse directly assists the Director of Residential Services (DRS) in the clinical management of The Garden, a memory care community. The Wellness Nurse exhibits a thorough understanding of the stages of dementia and the needs of each resident and their family as they move through these stages. The primary areas of responsibility are the provision of nursing care as needed, timely compliance with medical and nursing orders and documentation of resident progress and need for services. The Wellness Nurse demonstrates positive customer service through effective communication, family advocacy, and collaborative relationships with all members of the residential services team.

Responsibilities

  • Works with DRS to assess prospective residents
  • Develops service plans for new residents in collaboration with resident and family and in accordance with state regulations.
  • Keeps current on resident status, documenting at least monthly, and follows through with obtaining needed therapies and changes in care
  • Communicates with resident health care providers regarding medication orders and changes in health status
  • Triages resident health needs; provides nursing care, contacts family, refers to PCP, or transfers to hospital as appropriate
  • Works in collaboration with pharmacy services to ensure accuracy and safety of patient medication regime and medication administration processes. Participates in the medication destruction process per pharmacy protocol and state regulations.
  • Serves as resident and family advocate.
  • Demonstrates ability to effectively and calmly manage emergency situations.
  • Maintains age specific nursing competencies for adult and geriatric populations.
  • Participates in nursing on-call rotationProvides leadership for nursing staff by upholding resident-focused, person-centered standards of care
  • Assures nursing staff are kept informed of resident status and changes in a timely manner
  • Assists the DRS in monitoring and evaluating the quality and consistency of the nursing care provided to residents.
  • Presents positive image to the community and contributes to facility marketing efforts including tours and informational meetings with prospective families as appropriate.
  • Participates in the interview process of all potential new employees in collaboration with the DRS.
  • Coordinates the orientation of new staff to ensure each new employee has the knowledge and skills to maintain a high level of excellence in care delivery.
  • Helps to develop curriculum for and deliver required dementia training for new employees
  • Collaborates with Manager of Social Events and Volunteers to deliver monthly dementia education series for the public
  • Assists the DRS in clinical staff development and educational planning.
  • Contributes to employee performance evaluations both formally and informally.
  • Assists the DRS in the state survey process.
  • Encourages and promotes positive and professional relationships among all staff.
  • Oversees the department schedule to assure the maintenance of required staffing pattern on a daily basis, including input into the payroll system.

Qualifications

  • RN required, State of Maine
  • Prior experience with gerontological nursing, dementia, memory impairment strongly preferred.
  • Ability to function independently with minimal supervision
  • Excellent organizational and communication skills
  • Excellent interpersonal skills, especially related to residents, families, and staff
  • Strong computer skills and knowledge
  • Current CPR BLS Healthcare Provider certification.
